Extreme Packet Loss

I have extreme packet loss over 60% according to pingtest.net and that is a pretty huge deal. I've tried a lot of things like changing DNS servers, flushing the DNS, etc. I don't know what to do can anybody help me out with this issue? I can't play anything like League of Legends or games that require internet because it lags every couple of minutes. What's weird is sometimes it won't do that until a certain amount of time has passed. I'm willing to try anything.

Sounds like your ISP is throttling your data stream, you should contact them to find out if they are the cause first and if so, why?

Try running a tracert in command prompt to see what hop is causing the issue. 
Take screen shots and email them to your ISP tech support.

Ping 127.0.0.1 -t
ping your (network address) -t
ping google.com -t
allow to run each of them at least for 30 round and check the time difference between each rounds.
That will determine whether issue with NIC card, Modem/Router or Latency.
