is the disk in good condition sometimes if they are scratched it can cause a read fail, what type of disk are you trying to make the ISO from. one of the programs I use is called ImgBurn very good for making ISO,s from disk

Are you trying to create an .iso of a disk or disc? Your title says one but you question in the first post is asking another.
just a thought why are you making an ISO for what reason if you don't mind me asking, some member may say you can do it another way without having to make a iso but achieve the same goal, is the Disk a camcorder recording originally
