you really can,t avoid those issues
because a video driver can become corrupted
your video card can go bad /display

Issue could be related to
- Failed or failing graphics card
- Corrupt display drivers
- Failed or failing monitor
- Bent pins (Old machines and Monitors)
- Bad cable.
- Overheating. (try applying new thermal compound)
- Bad hardware.
