Make sure you plug it in the first DVI port. Check the pins for the right port. Example: plugging in the wrong DVI port can cause the issue: no video input/enter sleeping modeHope this helps you out.

Try the link below and if none work, try the list below. How to Troubleshoot a Vista or Windows 7 Sleep Mode Problemhttp://www.vistax64.com/tutorials/63567-power-options-sleep-mode-problems.html1. Try and disable sleep mode.
2. Check graphics drivers, (update them? if you have roll back to a older driver.)
3. Check to see if it happens in a live environment ie: puppy linux etc
4. Could be the Monitor or Cable from the Monitor (Change it and see if issues goes away)
5. Could be a bad slot on the motherboard?
6. Try updating Windows Drivers, Chipset Drivers.
7. Try Repair Install of Windows
8. Try and Flashing BIOS
9. Try Clearing Cmos & repalce CMOS Battery
10. Make sure you install driver for Graphics Card and don't let Windows install driver.
11. Are you using Wireless Keyboard and Mouse? (if so try Wired Keyboard & Mouse)

Well if you have tried everything in list then and check all other hardware "apart from Motherboard". Then it looks like Motherboard could be bad.

Its worth swapping out cables to make sure before you bite the bullet and buy a new motherboard.
